1) Is there such a thing as a bad inbound link? - Yes. If your site is linked from an ill-reputed site, it can count against your PageRank (”SEO score”)
2) Does a google add count as an inbound link? - I’m not quite sure what you mean by this one.
3) What is the optimum keyword content? 5%? - Also known as “keyword density”. My source says 3.14159265%.
4) Is there such a thing as excessive content, or content dilution? - I don’t think there can ever be too much content (excessive content). How do you mean content dilution? As in, covering too broad a topic/too many topics?
5) Which is better for internal navigation or does it matter?
a) index.php?pageName=whatever”
b) http:/myurl.index.php?pageName=whatever”
No solid facts have been settled on this. Google does seem to recognize the difference between /index.php and .
6) Should I make it a point to have fresh content on the index page, or should it be in the site map, or even at all? If so, how often? - Keep your site updated regulary and you’ll find yourself with a good chance of a good PageRank. The index page with fresh content as well as the other pages - although I recommend the index page primarily. A site map is a BRILLIANT idea, by the way!

I think you can get bad inbound links mostly paid links are frowned upon now, in my opinion as long as you write good content and update or add to it weekly then you will have no problems, you can spend hours worrying about percentages when in reality no one knows for certain and the criteria changes anyway.
I work normally concentrate on two things and they are good quality inbound links (must hold a page rank) and adding new content regular, I ensure my keyword density is acceptable for what I want and thats it.
With this I have managed to get a 3 month old domain to page rank 3 receiving tons of traffic and more than 20 top ten positions in google for different search terms.
My urls are all unfreindly to be honest as they are php and very long but I do keep my site’s sitemap updated weekly as the sites visitors are posting articles daily.
